If you wish to have your name added to the roll, you can download and print the form:

Paper copies are also available at both churches.

Do fill in a form if you are:

• new to the church
• did not fill in a form last year
• have moved into the Parish in the last year
• have now reached the age of 16

Please complete a form and return to the Clergy or Parish Office by Friday 2nd April 2010.
